With forms, your customers answer the questions that matter while they book and their answers show up right alongside the appointment automatically.
Whether you need additional appointment details, or just a heads-up on what someone wants out of a session, forms get you there before the appointment starts.

Creating a form (web)
Only account owners and admins can create, edit, delete, or assign forms on the web app.
Go to Settings > Forms. Select Create form if this is your first form, or + New form if you've created one before.
Give your form a name.
Choose which service(s) the form applies to.
Add your first question, then choose an answer type for it:
For short text or long text answers, toggle Required on if the customer must answer the question, or leave it off to make it optional, then select Add.
For single choice or multiple choice answers, enter the answer options you want your customer to choose from by clicking on the text field and typing an option, and select + Option if you want to add more choices. Then toggle Required on or off, and select Add.
Select + Question to add another question.
Select Preview at the top right to see how the form will look to your customers.
Select Save.
You can create as many forms as you need, and assign more than one form to the same service, for example, a general intake form plus a service-specific consent form.
If a service has multiple forms, you can set the order they appear in from the Services page.
Deleting a form (web)
Go to Settings > Forms.
Select the three dots next to the form, then select Delete.
You can also delete a form from inside the form editor by selecting the trash icon at the top right.
Editing a question (web)
Go to Settings > Forms. Click the form you would like to edit.
Select the question you want to change.
Make your edits, then select Save.
Reordering questions (web)
Go to Settings > Forms.
Hover over the six-dot handle on the left of the question you want to move. Click and hold, then drag the question to its new position and drop it.
Select Save.
Deleting a question (web)
Go to Settings > Forms.
Hover over the question you want to remove and select the trash icon on the right of the question.
Select Save.
Assigning a form to a service (web)
You can also attach or remove a form directly from a service:
Go to Settings > Services.
Open the service you want to edit.
Attach or detach the form(s) you want to use for that service.
If you have multiple forms, you can reorder them to choose how they appear on your Booking Page.
If a service has no form assigned, customers won't see a form step when booking that service.
Form character limits
These limits apply to every form you create. If a customer’s answer goes over the character limit, they won’t be able to type any more.
Limit | Value |
Questions per form | 20 |
Forms per service | No limit |
Form name | 255 characters |
Question text | 1,500 characters |
Short text answer | 500 characters |
Long text answer | 2,000 characters |
Options per single or multiple choice question | 1,500 |
Viewing and managing your responses
You can view and manage your responses across web and mobile. Once a form is submitted, the responses are attached automatically in two places:
On the appointment
Open any appointment to see the answers submitted for it. Responses here are read-only.
On the customer's profile
Account owners and admins can see a customer's full response history across all their past appointments, with each response linked to the form, service, and appointment it came from.
Things to note about your form responses
Responses are permanent.
Once submitted, a response cannot be edited by you or the customer. This keeps your records accurate for every appointment.
Editing a form doesn't change past responses.
If you update or delete a form, previously collected responses stay exactly as they were submitted.
Deleting a service or appointment doesn't delete the responses.
They remain accessible from the customer's profile.
